How Aged Care Speech Pathology works at Best Home Therapy

At Best Home Therapy, our aged care services are grounded in respect, compassion, and clinical expertise.

Here’s what you can expect when working with us:

Contact us by phone, email, or via a referral from a GP, nurse, or care coordinator. We’ll gather basic information about the resident’s needs, current health status, and preferred contact person so we can plan the next steps.

A thorough evaluation to assess communication and swallowing needs.

This also includes a process of collecting relevant medical history, medication lists, current diet/texture requirements, recent reports, and consent.

We review assessment findings with the resident, family and care staff and set meaningful, measurable goals (for example: safer mealtimes, clearer conversation, or improved intelligibility).

This will be a plan that summarises recommended strategies, supports and priorities for therapy.

Therapy is delivered in the home or facility and tailored to the individual’s needs. Sessions may include swallowing exercises and safe-feeding strategies, voice strengthening, speech clarity work, and/or cognitive-communication tasks.

We provide hands-on coaching for family members and care staff which covers safe mealtime routines, cueing and communication strategies, use of adaptive equipment, and how to embed therapy into daily care.

Progress is monitored and the plan is reviewed at agreed intervals. We update goals, adjust strategies, and provide outcome reports for the resident’s record and for family or facility files.

We work closely with families, GPs, nurses, and care staff to provide holistic, respectful support.

    Referrals, Funding, and Access Options

At Best Home Therapy, we are registered to deliver services under Home Care Packages (HCP). Additionally, we also accept Private Health Insurance if this option is more suitable for you. Our therapy is available through home visits, giving you flexible options to suit your needs.

Areas of Aged Care Speech Therapy

At Best Home Therapy, our speech pathologists provide personalised, in-home therapy designed to meet the unique needs of older adults. Some may require support with speech clarity or language, while others may experience challenges with voice, confidence, social interaction, or even eating and swallowing safely.

Below are some of the common areas where speech therapy can assist, helping to build confidence, improve communication, and enhance everyday life.

Ageing, dementia, or chronic illness can affect the muscles involved in chewing and swallowing.

Conditions like Alzheimer’s or Parkinson’s can affect speech clarity, memory, and understanding.

The natural ageing process may cause the voice to feel weaker, breathy, or more effortful.

We assist with strategies and equipment for safer, more enjoyable mealtimes in both home and care settings.

Whether it’s improving speech clarity, maintaining communication, strengthening social connections, or supporting safe swallowing, we focus on meaningful outcomes that promote confidence, dignity, and quality of life.
