Home Care Packages (HCP) for over 65-year-olds.
As our population ages the importance of staying independent and safe at home has never been higher. To keep people mobile, safe and happy in their homes as they age; HCP funding was created.
Best Home Therapy has been assisting in providing meaningful support to seniors in the community for years. HCP funding is a great tool that allows more of the community to access Allied Health services.

Funding from the federal government to assist seniors access care services within the home.
The amount of funding varies depending on the individuals care needs as determined through a needs assessment. Once a needs assessment is completed, allocation of a funding package from level 1 to level 4 will be given.
Funds are cumulative and can be utilised for a wide variety of services, such as physiotherapy.

HCP packages require the individual to be over the age of 65 and looking to keep their independent living within their home.
Anyone can apply that is assisting the individual with their care needs, this may be their family, or a HCP Care Manager who assist eligible people with their within the home care needs.

Those eligible for a HCP, i.e. people requiring funding to maintain independence at home, (this process can be triggered by a range of scenarios such as a hospital admission or being started by friends or family).
The eligible HCP individuals will be approved for an HCP package and then sign up with a Support Coordinator/provider.
The provider will be responsible for the assessment of the participants needs and then can refer service provider’s (such as a physio).
BHT will then liaise with all parties involved to organise the initial physio assessment and subsequent treatment plan.
BHT strongly values its collaborative relationship with those on HCP and the service provider, working as a team ensuring the best client outcomes.

STEP TWO: WELCOME
Once we have your contact details, our super friendly staff will get in contact with you or your support provider to organise the initial consultation.
STEP THREE: INITIAL CONSULTATION
The initial consultation is where we gather all the necessary information (medical history, goal setting, lifestyle, etc.) to create a comprehensive treatment plan to help you LIVE YOUR BEST LIFE.
STEP FOUR: TREATMENT PLAN
Our experienced and supportive staff will work collaboratively with you and your support worker to develop a specialised treatment plan that specifically targets what you need. Treatment plans include guided exercises, expert resources (e.g. sleep, nutrition), support and a multitude of other forms of therapy to achieve your health & wellness goals.
STEP FIVE: REVIEW
After 6 weeks, we will review the effectiveness of the treatment plan as a team. We want to ensure that we are providing the best service possible and encourage feedback throughout. Achieving your health & wellness goals is our priority.
